Create a short employee pulse Google Form

A short team pulse can help identify one obstacle to discuss and one change to try. This example uses two practical rating questions you can repeat each round; it is not a validated engagement scale or an employee assessment.

About this example

  • Four questions, with no name or email field in the example. Google Form settings still need review.
  • Use the same scale and timing for repeat rounds. Interpret changes alongside discussion; do not present this example as a validated score.

Before you start

  • Explain the purpose, who will read responses and how the team will act on them.
  • In Google Forms, check Collect email addresses and account restrictions. Turn off email collection unless you need it; leaving names out does not guarantee anonymity.
  • Consider whether a small team or a detailed free-text answer could identify a respondent. Invite people to omit names and sensitive details.

Create your form

  1. Open the example and choose a consistent reporting period, such as this week.
  2. Keep the two scale questions and their anchors consistent for future rounds; preview the text before creation.
  3. Create the Google Form, review privacy and access settings, and test the respondent view before sharing. Discuss one aggregated obstacle and one next action.

Limits and settings to check

  • This is a practical pulse example, not a validated diagnostic instrument, HR score or individual performance test.
  • SurveysAI does not guarantee anonymity or aggregate responses. Identification risks depend on settings, group size and the content people enter.
  • Do not use the example to collect sensitive employee information or decide individual employment outcomes.
