Create a product order request Google Form
Use this form to receive an order request you will confirm manually. A response is not a completed purchase: availability, price and fulfilment still need a separate confirmation.

About this example
- Six questions; the first line of the introduction makes the request/confirmation distinction explicit.
- Check quantities and contact details manually before confirming availability.
Before you start
- Replace Product A and Product B with the real choices and explain that price and availability will be confirmed separately.
- Choose how you will contact the requester and what lead time you can realistically communicate.
- Decide who will check requests, confirm quantities and handle payment outside the form.
Create your form
- Open the example and adapt the product choices, introduction and collection arrangements.
- Preview all six questions, including the requested collection date, then create the Google Form in your account.
- Review each response, check availability and contact the requester to confirm terms before treating it as an order.
Limits and settings to check
- The form does not take payment, calculate a total, reserve stock or confirm an order automatically.
- Quantity is a text field; validate it yourself. A requested collection date is not a guaranteed pickup appointment.
- Use a dedicated commerce checkout if you need immediate payment, tax calculations or inventory control.
