About this example
- Five questions. Ratings describe different course dimensions, not a combined validated score.
- Keep future evaluations comparable by retaining the same wording and scale.
Before you start
- State which course and delivery period the evaluation covers.
- Check whether your school requires its own evaluation system; use that process when required.
- Explain who will read responses. Review email collection and account restrictions before suggesting that answers can be anonymous.
Create your form
- Open the example and add the course name and reporting period. Keep the scale anchors consistent.
- Preview the three rating questions and two open questions, then create the Google Form in your account.
- Check the respondent view and institution requirements before sharing. Compare themes across answers and choose one course improvement yourself.
Limits and settings to check
- This is a practical example, not an accredited evaluation or a validated educational measure.
- Leaving the name out does not guarantee anonymity. Settings and identifiable free-text details still matter.
- The generator creates questions; it does not score instructors or analyse responses.
